Post Shipping Logic Review 
Hello again I am hoping that someone can review my custom shipping logic and let me know if it is setup ok as I am not sure how to test it within my store. Since I intend on using the Paypal gateway I can complete that part of setup until I pay for my membership.

Shipping Logic:

"Continental US Ground|1-24.99|||6.50",
"Continental US Ground|25-499.99|||25%",
"Continental US Ground|500-|||11%",
"Continental US Rush|1-24.99|||20.00",
"Continental US Rush|25-499.99|||65%",
"Continental US Rush|500-|||60%",
"AK, VI, HI, GU, PR|1-24.99|||12.50",
"AK, VI, HI, GU, PR|25-|||50%",
"Canada Post|1-24.99|||7.50",
"Canada Post|25-|||40%",
"Canada DHL|1-24.99|||20.00",
"Canada DHL|25-|||80%");

Your order form isn't setup.
Make sure that the options in the order form match the shipping methods you are using, exactly.
However, this part will not work:
"AK, VI, HI, GU, PR|1-24.99|||12.50",
"AK, VI, HI, GU, PR|25-|||50%",

Post Order Form Updated 
Thanks for your reply I have completed modifying my order form to the way I want it to look. I intentionally broke the form to make it look exactly the way I want. Is there a way to have the order form look as it does currently on my site but actually have it work?

Don't put any of the header stuff in the orderform, the cart will automatically do that for you.
But, you can put the stuff that goes between the <head></head> tags in and edit the <body> tag.
Do not remove the agorascript stuff from the page, the script needs that stuff.
However, you can change around the layout of the displayed items with no problem.

DO NOT MOVE or REMOVE the following line in the file:
<H2>CART CONTENTS HERE</H2>

Make sure you set up the store urls in both the:
manager -> Payment Gateway
AND
manager -> Program Settings -> Main AgoraCart Store Settings
