If all you want is to change the Offline form try the following. Be sure you have a clean backup of the form first.
Edit the Oflline order form to remove Purchase Order, Check and Credit Card entries.
Remove or comment out everything between:

Add text at the top of the form stating that it is for COD and when they get to the final step to print the form for their records, or some such thing.
