Let me know what is wrong.

This is my store URL

If you try to checkout it only shows state and zip. That's as far as it goes.
and help would be much appriciated

Please do not post the same question in multiple areas.
Also, when posting your store's URL do not include the cart_id.

You store is calculating shipping.
On the first screen that asks for the shipping method, state and zip, you have to click on the "next" button for the shipping to show up on the next screen.
There is no way for the cart to know how to claculate the shipping without method, state & zip being inputted.

Post No shipping address 
where is the shipping address input supposed to be for the customer. I still don't see it in the form, even on the next page.

Did you see it after clicking next?

The customer info and credit card info is collected on Authorize.net's secure order form which comes up after the shipping is calculated.
