two questions in adding to product page

Post two questions in adding to product page 
I have omitted the quantity but I still have the button , which is ok i think. I want to click the button, which which will pull up a application (I think I can put that into the forms directory) Then I want to have the form go back to the page. Now I am going to try and use matt's script to make this form or do I need to do another type of script?

As you can see I am not to good in the web stuff but I am trying sorry if my language is not the correct to use in describing what I want to do.

What, exactly, are you trying to do?

Which of Matt's scripts are you trying to use???
If it is formmail, DON'T... It is a script that many web hosts disable because of the amount of spam that spammers can use it for. We use ennyforms which is a much better option to keep the spammers away.

Ok I won't use the matt script stuff. What I have done is the off line order form I have tried to modify it but I can't seem to pull it up in the main agora settings, to make it a default. As I have renamed the script credit.html ( I didn't want to mess up the original so thought that the program would just add to the drop down list to choose, but it didn't. So I guess I must have to add some script some where else to be able choose it main agroa store setting in the manager.

What i want to do is in the car site after the customer picks a car the can send in a form with credit information or as for help.

Oh, boy.... I have NO IDEA why you are trying to drop that form into a ppinc display????
This makes no sense.... use the system the way it is coded to be used...
However, you may need a lot of custom coding depending on, exactly, how you are trying to make this work...
To call the order form, it MUST be named Offline-orderform.html and be run through the system.
Just rename the original one to Offline-orderform-bak.html to retain a backup.

Now, in order to do all of the adding new fields to the form, you will, also, need to edit the Offline-order_lib.pl file... This is where everything gets pretty tricky, but I have some instructions at:
