You will probably need to edit the AuthorizeNet-order_lib.pl file, the required fields area.

Yes, I've done that. Here is the code in my file:

On the required fields, make sure you have the final semi colon on the end like this:
@sc_order_form_required_fields = ();

Without it, could cause an error.

Make sure you have backed up the original file and then test it out.

It looks like you haven't edited the orderform.html file. You need to edit that, too, in order to remove the fields.
