UPS module error: shipping cost not determined!

Post UPS module error: shipping cost not determined! 
I had the shipping at https://www.camecousa.com/store/agora.cgi set to use custom logic that worked great till they changed their product line. Now we want to use the custom logic and the UPS calculator but no matter what I set everything to, custom logic, no custom logic, any UPS settings will give an error:

UPS module error: shipping cost not determined!
Please use your 'Make Changes' button and correct
any fields as required and try again.
Please contact the site administrator ASAP if error continues

And the customer can then go no further in the sale.
So I have spent 2 days on the support and forum pages and still can't get past this error. Please tell me this is something simple and stupid:)

Rick,
You're gonna have to be more specific than that...
We need to know exactly what you have set in the Shipping Settings manager...

Also, it appears that the store is currently NOT set up to even use UPS, at least it is not an option in the order form...
One other thing, have you made sure that all of the AgoraCart files are up to date?
You can check on that in the "Upgrade AgoraCart Modules" manager.

Hello Bonnie,

I had hoped this would be an easy fix that you had seen previously. At any rate, I did the update that you mentioned and for a number of the files it fails the update and says "Updates Attempted: ERROR: The file you requested was not reachable for updates.". It doesn't say whether the problem is on the Agora server or my site server. This also happens with the ship_settings-ext_lib.pl file. With any of the files that say there is a 5.2.xxx update, it says it updates but then when I look at it again, it is the old file.
But more info on the UPS problem. I have to reset the shipping logic when I am done otherwise no sales can be made becuase it will not move past the UPS error. I tried removing custom logic. So the latest attempt looks like this:
